Option 2
Paper Registration
Follow these three steps to register:
Step 1
Get a pledge form. There are several ways to receive a pledge
form:
- Option 1: Start to collect pledges
today and
download a pledge form (PDF format;
Acrobat Reader required);
- Please call 1-716-684-0640
and have a form mailed to you.
Step 2
Contact all of your friends, family and co-workers asking
them to sponsor you in the Tiptoe through the Tulips for
Parkinson’s 2008.
Step 3
You can register in 2 different ways:
Register in advance of the walk date for your area by calling
the Parkinson Association office at (716)668-8505
Register on the day of the walk by bringing in your completed
pledge form and collected pledges to the registration table.
Fundraising tip: Many companies offer a matching
gift employee benefit program. Check with your human
resources department and see if your employer offers this
benefit and double your contribution in one easy step!
